Job Description

- The role of Domiciliary Care Coordinator involves the coordination the delivery of Domiciliary Care for disabled children and young people across Lincolnshire. Primarily, to manage the Domiciliary Care team and support the Area Manager to ensure services meet the requirements of the Care Quality Commission (CQC).Key responsibilities :To coordinate the domiciliary care rota across Lincolnshire ensuring families receive the care at the times they need it.To support the recruitment and retention of staff deliver domiciliary care across the country.To manage the challenges of staff absence and annual leave ensuring appropriate domiciliary care cover is prioritised to those most in need.To undertake home visits and complete care plans, risk assessments for children and young people referred to the service.To maintain regular contact with lead professionals referring to the service.To foster relationships with domiciliary care agencies to ensure emergency cover can be provided.
