Job Description

We are looking to provide flexible support to care for people of all ages to help prevent inappropriate admissions to hospitals or nursing/residential homes. You will be working along side other professionals such as; DN's, Therapist's, GPs and Social Workers to help settle service users who are returning home. This will involve encouraging service users to regain their confidence and strength in the comfort of their own home. We are looking to recruit people who are flexible and who can respond to urgent situations. We need experienced staff that are able to undertake a range of care tasks if needed and to have the ability to promote independence. Experience working with a wide range of disabilities is essential as well as excellent communication skills. You must be able to provide hands on support to assist with managing personal care, meal and drink preparation and mobility support. You will also need to have the ability and skills to encourage and build confidence to enable the service user to become fully independent, hopefully preventing the need for on going support wherever possible. We are looking for people who can provide a short term intervention to help aid and support service users. This will include covering an absence of a regular carer, providing some support to someone being discharged from hospital and the prevention of a further crisis. You will work with service users from 1-6 weeks to help provide continuity and to maximise potential of reducing the need of longer term support.
